(1) Discuss the management of a case with a prosthetic heart valve and low grade fever.

(30 marks).

(2) What are the factors that raise the possibility of :

       A- Renal artery stenosis.

(20 marks).

       B- Pulmonary embolism.

(20 marks).

(3) Discuss the management of each of the following:
       A- Torsades de pointes.

(15 marks).

       B- Chest pain for 2 hours and ST depression.

(1) A 75 years old patient complaining of dyspnea grade II/IV, recurrent palpitation and occasional exertional angina. Echocardiography showed; moderate eccentric LV hypertrophy, sclerotic moderately calcified aortic valve with a peak systolic gradient of 30 mmHg.
    - Discuss the following:

       A- Investigations needed.

       B- Strategy of management.

(20 marks)

(2) Discuss each of the following:

       A- Complications of heparin therapy.

(20 marks)

       B- Indications of primary PTCA.

(20 marks)

       C- Long-term complications of total correction of tetralogy of Fallot.
