(1) Describe the management and means to assess myocardial viability in a coronary heart disease patient with impaired systolic function.

(30 marks).

 

(2) Discuss each of the following:

      A. Serum Ca alteration on surface ECG.

(20 marks).

      B. Resistant systemic hypertension.

(20 marks).

(3) Discuss the management of each of the following:
      A- Acute pericarditis.

(15 marks).

      B- Recurrent deep venous thrombosis.

(15 marks).

Answer all questions

Time: 3 hours


(1) A 62 years old male was seen in the emergency department (ED) with30 minute duration mid-precordial chest pain. It was radiating to the medial aspect of the left forearm and was relieved with repeated sublingual nitroglycerin.

On examination; his pulse was 92 bpm, BP 140/90 mmHg, and there were no signs of cardiac enlargement or heart failure. The ECG showed old inferior wall myocardial infarction and flat-inverted T-wave in V4-6.

- Discuss the immediate management and future investigation of this case.

(30 marks)

(2) Give a short account on each of the following:

       A- Arrhythmogenic RV dysplasia.

(15 marks)

       B- Cardiac complications of blunt chest trauma.

(15 marks)

       C- Cardiovascular indications for legal abortion.

(20 marks)

       D- Echocardiographic predictors of successful balloon mitral valvuloplasty.

(20 marks)
