     a- A 55 year old diabetic female patient presenting with shortness of breath and complete left bundle branch block.

     b- The recent development of congestive heart failure in a 35 year old patient ; 5 years following successful mitral valve replacement for rheumatic heart disease.

3) A 45 year old male patient received radiotherapy to the mediastinum for treatment of Hodjkin’s disease in 1991. His condition remained stable until four months ago when he noticed the development of palpitation, facial swelling on waking up and progressive oedema which started in the legs and marched upwards. Other findings on examination included sinus tachycardia 105/min, BP 110/85 mmHg, JVP was elevated 8 cm showing prominent X descent. The liver was enlarged 3 fingers. Chest showed diminished breath sounds at the lung bases. Cardiac auscultation revealed an S3 sound. The ECG showed sinus tachycardia, low voltage and nonspecific ST-T changes. Chest x-ray showed mild cardiomegaly and small bilateral pleural effusions. Blood picture was normal, serum creatinine 0.9 mg/dl, AST 26 U/L, ALT 19 U/L, serum albumin 3.4 Gm/dL and urine protein 0.14 Gm/24 hours.

a- What are the other investigations that should be obtained in this patient.

b- Discuss the diagnosis and differential diagnosis of this case.
